How to Use This Protocol Library
Come here when you're prepping for a visit and need a form or handout fast. Each protocol below has everything you need in one place — download and print what you need before you go.
Click the blue folder to open a protocol's forms and handouts. Each one is sorted into the form itself, Required Handouts, and Optional Handouts, with English and Spanish versions where available. Print the language your patient prefers.
The forms and handouts here are the same ones built into that protocol's training module — you don't need to visit both. If you want a refresher on how to use a protocol, click the green book to jump to its training module.
When to Use the Resource Library
For referral or other resources to share with patients, check the Resource Library instead.
The New Patient Intake Form gathers demographic, clinical, utilization, and care-planning information before the first Connected Care visit. This information helps the team assess urgency, select appropriate protocols, coordinate necessary supports, and tailor the visit to the patient’s needs, preferences, and circumstances.
“What Matters” is one of the foundations of age-friendly care. It begins by getting to know the person before focusing on decisions or services.
The Health-Related Social Needs (HRSN) Assessment is a screening tool CHWs use in the first few visits to rate a patient across six social-need domains (like food, housing, and transportation). This assessment helps identify which needs require a referral.
An Advance Directive is a legal document for adults (18+). It allows a person to express their wishes for end-of-life care and to designate someone they trust to make healthcare decisions on their behalf if they become unable to do so.
