Developed by the Columbia Gorge Health Council, a nonprofit organization that partners to deliver services to patients in rural Hood River and Wasco Counties, Oregon.
About Connected Care for Older Adults
Connected Care for Older Adults (CCOA) embeds community health workers (CHWs) in rural primary care clinics to deliver age-friendly care to adults 55 and older who are experiencing increasing frailty. The program is built around the 4Ms framework β What Matters, Medication, Mentation, and Mobility β and is designed to extend clinical capacity to better serve older adults.
CHWs work directly with patients to understand their goals, connect them to services and supports, and flag concerns back to the care team.
